Yeah it's in a home...but it is a bit odd for sure.
I know that such showers have been built in private homes occasionally — LBJ had one in his Texas home and, I think, in the White House; there's a story about it — and maybe, yes, it's a rich person's toy to be operated by a servant. I'd still bet a wooden nickel it was installed for someone who needed special care and the family has the means to provide it.

I've seen some fancier versions that had curved horizontal pipes and even more shower heads. Wonder how they got enough water pressure to run that many shower heads all at once? Perhaps the valet would only operate one or two at a time?

That's more or less the LBJ story. He wanted a super-shower, which somehow became the Secret Service's problem (and expense). And then his ranch didn't have enough pressure, which became the Secret Service's problem (and huge expense, for a pressurized water system).
So one day he calls his aide into the bathroom... LBJ in his birthday suit and the aide in military uniform. He complains that the shower isn't working. Aide steps in, turns handle, gets soaked to the skin in two seconds. Steps out. "Well," LBJ smirked, "It didn't work for me."
I like a good handheld, and I've experimented with the multi-level showers on the luxe decks of cruise ships, but I really can't see the need for such a shower or ignore the huge waste of water...
We had a multi-head system with a "rain" feature overhead. My wife loved it. I hated it when all of them were turned on. Felt like you were being waterboarded. I greatly preferred the guest bath with one regular shower head.
No, it was operated by one's valet. Only the very wealthy people had these installed in their homes, and they didn't wash themselves. You all have overactive imaginations.

You can regulate the pressure or switch off selected heads. It's a great massage for your body...
