No boxes are included with Fios. I have a regular HD box for which I pay $5.99/mo., and an HD DVR for which I pay $15.99/mo.
Even the regular SD boxes have a $4.99 or $5.99 price tag.

The deal I have is $90 for the first 3 months, and $116 for months 4-24, with 1 year agreement and $150 cash back. I have TV Extreme and 15/5 Internet, 1 HD set top box, 1 standard set top box, and 1 HD home media DVR.

Throw in Ooma for free monthly telephone (which is absolutely fantastic, I have it now for a few weeks), and I'm relatively satisfied with what I have and pay per month. Now I am still looking for an internet-based TV that is competitive with what I can get through a cable company, and I will be even much happier!
You failed to mention the start up cost of your "free monthly service" which has a start up cost of $249.99.

Taken from their website;

"How much does Ooma cost?
The Ooma Hub bundle has a list price of $249.99. This includes an Ooma Hub, Ooma Scout, and a 60-day free-trial of Ooma Premier. Ooma Telo has list price of $249.99 and a 60-day free-trial of Ooma Premier."
