Got my email today too. Little weird navigating through it but I assume I filled all 3 parts out correctly...no need for the Visa part. There was no confirmation email or notification on page when and if you were done.Guess they have us sign it when we go in

yeah you always check INTERSTATE. if you have an A3 restriction it's the third one like he said, then sign it and mail it in as is.

Those with no A3 restriction, check the first box, sign it, INCLUDE YOUR MEDICAL CERTIFICATE, and mail it in.


If you have the K restriction....go get it off now
