I think you have misconceptions as to what would actually play out...how nukes actually work and what is "winnable".
Most of the warheads are in kiloton and a few in megaton...the Tzar Bombas (50/100megaton) were novelty and not anywhere near practical for tactical purposes...
If I remember correctly I read that the Satan 2 missile could carry nukes that are 400x more powerful than those dropped on Japan.
Most nukes deployed by most nations today are 400x more powerful than those dropped on Japan in WWII.
The Hiroshima and Nagasaki bombs were pretty dinky as modern nukes go. They were measured in the 15-25 kiloton range. Modern nukes are in the megaton range.
Still, there's no such thing as a "weak" nuclear bomb, including the 1945 ones. Even those "comparatively small" ones wiped out half a city each, and killed hundreds of thousands. Modern ones can take out an entire LARGE city plus a lot of the area surrounding it. And many nations have missiles that can carry many nukes at a time and drop them on multiple targets.
Some people seem to be forgetting MAD (Mutually Assured Destruction). Russia's nuclear arsenals might not be as advanced as the USA, but they can still destroy us because we can not destroy all their nukes in a first strike. And our missile defense system can only intercept a small percentage of incoming missiles.

Russian submarines use a new tactic.
They leave a container with a nuclear torpedo somewhere on a sea floor. It can stay there for years. If a signal to cancel the mission doesn't come, it swims slowly to a programmed location near the enemy's shore and detonates a nuclear bomb creating a 0.5-mile-high tsunami.
