Sure it does. Personal details such as whether or not an individual has been vaccinated against COVID-19 comes within the “provision of care” classification of health information that should be protected when it can be combined with other personal details (i.e., name, social security number, etc.) that can identify the individual. Protected health information is commonly referred to as PHI.
The HIPAA Privacy Rule limits uses and disclosures of individuals’ PHI to uses and disclosures required for treatment, payment, or healthcare operations.
Neither school enrollment, admission to venues, nor employment, etc., meets the qualifications for exceptions to HIPAA.
HIPAA doesn't apply to you disclosing your own health information. It only applies to providers of healthcare and entities involved in providing or paying for healthcare. If you give permission, then the data can be disclosed.
I can demand that you provide proof of vaccination, and refuse to provide services if you fail to show me you are vaccinated. No violation of HIPAA there. If I call your doctor and ask "Is InformedConsent vaccinated" and they answer, then your doctor has violated HIPAA.
