It depends on the man and the particular moment. When I saw my BF's tiny profile photo I thought he was very handsome and had a fantastic smile. His photos showed his playful side, and then I saw more photos and yep, definitely delicious.
When I saw him in person for the first time I had the "He's f*cking HOT!" moment. Gorgeous, warm and infectious smile. "Hot damn!" I was thinking. When I'm really drawn to someone on multiple levels, my mind immediately goes to kissing, especially when he has nice lips. Lips are incredibly important to me because nice lips make the kissing experience deliciously hot and fun. Kissing, the act, is very primal, and it tells me a lot about a man, how attentive and giving he is, how receptive he is, his level of passion and intimacy. It is probably my favorite form of physical intimacy, so my mind naturally wanders there when I'm sexually attracted to someone. It is the basis, along with touch, of physical intimacy.
So, if we're referring to a specific sexual act or thought, it would be kissing. Though other times my thoughts are far more explicit.
At first glance, I don't think or do much except perhaps a double take, or a "whoa, hell yeah" moment. I generally don't think about sex...I'm kind of boring in that regard, I guess.

I am glad I have a vivid imagination .. sometimes it even makes my toes curl!
When I was married, though, I noticed good looking men, but I must have thought of all of them as eunochs because I didn't imagine having any kind of sex with them. Either that or my husband was keeping me plenty happy, which is probably the case.
