I wouldn't fly any African airline, Aeroflot, Air China, and many of the cheap-o Asian carriers.
I also wouldn't fly Air France again, because their seats and service suck.
Korean Air wouldn't be on my no-fly list, and probably not Malaysian, although I think Cathay Pacific flies to most of the same places and is a much better carrier.
I know this will be taken as racist, but I am not PC and don't care if the easily offended crowd does not care for it.
A friend and quality pilot for many years with a major carrier says he cannot believe how poorly trained many other countries pilots are, and frankly how dumb. He points to Asian pilots as being poor, though it is not because they are dumb.
African and Latin American airlines are ones he will never fly on for example, and few from Asia either.

I agree. I didn't take the comments as racist but rather based in truth. The biggest problem seems to be with the comprehension and/or pronunciation of the English language--which is the standard for all flight communications in the world. It's a criteria I also use when choosing a doctor. If I can't understand them and they can't understand me, I don't care how good they are, I'm putting my life at risk, so I'll go elsewhere.
By the way, one of my doctors is male and Muslim and another is female and Chinese--both have a very slight accent and we can clearly communicate. It has nothing to do with their race or country of origin and everything to do with the ability to effectively communicate.
I can't say that there is an airline I don't trust. I won't fly on Air Tran, but that's due to incompetence and poor customer service. From your list I have flown on Thai and Malaysian airlines. Malaysian wasn't bad, Thai airlines was pretty good and worth flying again.

I don't think this is racist at all. There is a chapter in Malcolm Gladwell's book Outliers that examines cultural dynamics of flight crews and their relation to accidents. In the US a co-pilot would tell the pilot "hey, you missed this" but in other cultures it's not the co-pilot's "place" to directly point out the pilot's mistake and that contributes to crashes and the resulting poor safety records.
For myself, I try not to fly AirTran because they are the "remains" of ValuJet. After the crash of ValuJet 592 near Miami the airline merged with AirTran, taking the other company's name because their own brand was fatally tainted. Since AirTran is in the process of merging with Southwest they're probably perfectly OK to fly but I know many first responders who worked the flight 592 crash scene so my apprehensiveness at flying AirTran is probably because of the stories I've heard.
Also Ariana (the national airline of Afghanistan). Government employees and contractors are actually forbidden to fly Ariana, but the airline is also not allowed to operate in the EU so most people will probably never deal with them.
And finally United, because I just never seem to have a good flight with them.
The only bad experience I had on Air France, was changing planes at Charles De Gaulle which was a pain in the a** due to the confusing layout. I never had any bad experiences with either the plane or crew itself.
Anyways probably the only carriers I will never fly with are Air Koryo, Iran Air, Iraqi Airways, or any air carriers banned in the European Union/US due to safety concerns.
