That's just the way this show is, I tend to think of it as lazy writing that they can't create dramatic situations, and still be somewhat realistic, but I suppose it may be a budget issue. I still enjoy the show, so I usually just let it go, but it does annoy me a bit when it's so obvious.

They don't do a bad job on the accents. I've watched all the actors in other shows and I never noticed an accent except Bracco's, so that must mean they all do a Mass./Boston accent pretty well.
