Spain hands down. Same religion. Very similar culture, language, economy and both have a lot of shared history. They were united for several centuries, starting with the Roman Empire and ending with the Spanish Empire, much of today's Italy was part of the Greater Aragonese Empire.
I am from Barcelona and the first time I went to Rome I was shocked to feel like home. Barcelona is filled with Italians and most I know feel like home in here. Both of our countries are so similar in each possible way maybe except for the far north in both (Northern Spain is more Atlantic with Celtic roots, Northern Italy is more Central European) but I'd say 80% of Spain and Italy are almost identical.
People look very similar, the genes are very similar, the languages are very similar, the way of life is very similar, the way of behaving is very similar, culture is very similar, economy is very similar and even the architecture and the urban development of both countries is very similar. We even share the speaking loud feature!! Both countries are filled with cobbled narrow streets in the old towns and typical houses look similar. Even the geography and the climate of both countries is so similar it's scary to think they're different countries. Both are extremely rugged, mountains are everywhere as well as coastlines. People tend to think Portugal is similar to Spain and France to Italy but they aren't too much in reality.
