Observed cooked fried rice at internal temperature of 76F and cooked beef/pasta at 74F on front self serve table. Observed customers in the establishment/game room self serve themselves at the front counter table. Corrective action: provide time as public health documentation or provide a hot holding device to ensure a food safety holding temperature of 135F and above. Food voluntary destroyed.
Cross-contamination of Raw/cooked foods/Other Food must be protected from cross contamination. Store raw eggs below cooked foods inside reach in cooler. Observed raw shell eggs stored on top shelving above cooked pans of rice inside kitchen area reach in cooler. Corrective action: cooked rice moved to top shelf and will not be served to consumers until permit is obtained.
