Observation: Hot water sanitize did not turn test strip black indicating food contact surfaces are being sanitized. Pressure gauge didn't work - and backflow valve was leaking water.
- Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours
Observation: Crab asparagus soup made at 10:00am was 100 degrees in cooler. Discarded at time of inspection. Brisuct at 46 degrees. Discarded at time of inspection.
- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation: Pulled pork dated 3-15-15 in meat cooler. Discarded at time of inspectyion.
- Physical facilities maintained in good repair
Observation: Front of house wait station is being repaired, ceiling tiles are being replaced, walls are being painted and floor is still in process of being repaired.

Observation: Tomatoes, cheese, lettuce, mashed potatoes in make table holding at 50 degrees.

Observation: Handle on freezer is loose. Lid to big ice machine is broken.

Observation: Large ice machine has black buildup on baffle.

Observation: Potato skins at 46 degrees, discarded a time of inspection. Country gravy at 100 degrees- leftover from breakfast - transferred to shallow pans at time of inspection.
- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: Baseboards, conduit, drains, legs of equipment, outlets, painted surfaces are not clean to sight or touch.
- Beverage tubing, cold plates, and ice unit drain lines
Observation: Cold plates are not separated from drink ice. Metal pans are used to separate ice - cold plates could be upgraded with integrated cold plates.
- Service sink required
Observation: Mop water is being dumped down culinary sink. No mop sink in establishment.

Observation: Ceiling tiles with peeling paint, floor tiles missing and cracked, areas of establishment showing old age and are no longer easily cleanable in but not limited to kitchen, waitress area, bar areas.

Observation: Shelving and floors in dry storage had spilled food debris.
- Cleanability of floors, walls, and ceilings
Observation: Walls/floors/ceilings that are showing ware and are no longer easily cleanable - under bar sinks, under soda station at waitress station. Tiles that are broken.

Walls and ceiling are sioled/not easily cleanable - some tiles are missing and or broken which do not aid in cleaning. Tiles in walkin meat cooler are soiled and grout is stained.

Salad cooler holding at 47 degrees.
- Foods are cooled using appropriate methods
Leftover baked potatoes are not being cooled - observed stacked holding at 50 degrees.

Cold plates are not integrated into ice bins and are not easily cleanable.
