Observation: Use of cardboard to line shelves in refrigerator is prohibited.

Observation: basement prep are handsink needs to be stocked with handsoap.

Observation: Reuse of single service containers.

Observation: Garage door in basement is not fully closed and has an excessive door gap which could allow pests to enter establishment.

Observation: Establishment needs a certified food protection manager. Item was marked at last inspection and was required to be complied with in six months from last inspection.

Observation: Meatloaf date marked 11/29/15 in deli refrigerator 12/15/15. Staff stated product had been frozen however product was not marked in a way to indicate when it was frozen or thawed. Product was discarded at inspection.

Observation: Cooked and cooled corned beef in walk-in refrigerator cooked 12/12/15 held refrigerated longer than 24 hours and not datemarked. Product was discarded at inspection.

Observation: Carry out heat and serve italian beef, BBQ pulled pork, and sausages in retail freezer are not labeled with ingredients. Labeling has been an ongoing violation.
- Maintaining premises free of litter and unnecessary equipment
Observation: Basement has a large accumulation or various items which are not used for the day to day operations.Basement needs a complete reorganization to eliminate unnecessary items and protect against pest infestation.

Observation: Drying mops in stairwell to basement.

Observation: Italian beef, BBQ pork, and sausages are not labeled with major allergens.
- Cleaning of maintenance tools and mop water disposal
Observation: Acess to mop basin was blocked by boxes at inspection.
- Plumbing system maintained in good repair
Observation: Condensate is leaking on chiller line in basement. Faucet on three compartment sink in the basement was non-functional at inspection.
- Preventing contamination from equipment, utensils, and linens
Observation: Meat loaf which was partially unwrapped was in contact with rusty refrigerator shelf. Meat loaf was discarded at inspection.

Observation: maintain wiping cloths in sanitizer between use.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: Storage of bags of raw ground beef above cooked italian beef in walk-in. Product was moved at inspection.

Observation: Numerous deli salads repacked in house or modified and repackaged in house were held for sale longer than 7 days. Products included seafood pasta salad, potato salad, white macaroni salad, and a second kind of potato salad.

Observation:Many packaged foods such as meals to go and deli sandwiches and multi-ingredient meats are not labeled as to contents. This has been a reoccurring violation and shall be corrected
