Observation: Leak observed under the middle three compartment sink basin at the main bar. Leak observed from the pipes under the white hand sink at the upstairs double bar room. Leak observed from the far right three compartment sink pipes at the upstairs double bar room. Repair plumbing leaks.

Observation: Drain lines missing from soda gun dispensers at all bars throughout the establishment. Re-attach drain lines. Observed back exterior door leading outside to dumpster area had gaps under the door with light shining through. Observed 2 dead cock roaches at upstairs bar area. Person in charge discarded roaches. Corrected.

Observed fruit flies in bottle of Amarreto. Person in charge voluntarily discarded bottle. Corrected. Observed black specks and residue on ice cubes in the ice machines located in basement. Person in charge removed all ice from ice machine to clean/sanitize interior of ice machine on this day. Remaining ice machine will not be used until all ice is disposed and interior of machine is cleaned/sanitized.
