3-601.11 Cease the sale of food not labeled for individual retail sale as specified in law. Cups of mixed fruit, Kool-Aid juice jammers juice pouches, and sleeves of Oreo cookies. The following shelf stable items were found to be past the manufacturers date limitation: Cambpells Cream of Celery soup expired 09/21/11 and Chef Boyardees Mac N Cheese made with real cheddar cheese expired 11/09/11 and 05/19/12. Items were removed from shelving and boxed to be returned to the vendor prior to the end of the inspection.
3-305.11 Store food at least 6 inches above the floor. Cases of Armour vienna sausages stored on the floor.
4-601.11 Defrost and clean bottoms of RI ice cream freezer. 4-602.13 Clean non food contact surfaces as often as necessary to prevent accumulation of residue. Accumulation of dust and grime on fan covers of RI coolers and freezers. Accumulation of grime and residue on fan vent cover inside of WI cooler. 6-301.11 Provide soap at all hand sinks. 6-301.12 Provide paper towels or drying device at all hand sinks. 6-501.18 Clean handsinks as often as necessary to keep them clean. Accumulation of residue and grime inside of handwashing sink.
6-501.11 Repair or replace damaged base and floor tiles. Damaged or missing base and floor tile throughout the facility, especially in the restrooms. 6-501.12 Clean ceiling and/or wall vent covers. Accumulation of dust and grime on ceiling vent covers. 6-501.12 Clean facilities (floors, walls, ceilings, fixtures, etc.) as often as necessary to maintain cleanliness. 6-304.11 Provide mechanical ventilation to keep rooms free of excessive heat, steam, condensation, vapors, smoke, fumes, and obnoxious odors. Exhaust fan inside of female restroom not functional. Extremely offensive stench coming from the restroom. This has been an ongoing problem for more than 2 years. Facility claims that the plumber has been called. Problem is possibly coming from the exhaust vent pipe/stack.
