KDA_3_30212 - --There were 5 pans of dry spices sitting in working pans with no labels. Located in the preparation area there was a bottle of yellow liquid with a label that said vodka. PIC said it was actually white wine. COS. Bottle was properly labeled.

KDA_3_50118A2 - --Located in the WIC there was one gallon of open milk with no date. PIC said it was open on October 20. COS. Milk was dated. Located in the bar area there was one open gallon of milk with no date. Employee said it was opened on October 18. COS. Milk was dated.
KDA_4_20211A2 - --Located in the storage room there was one large plastic container and 5 plastic lids with cracks. COS. Items were placed in trash can. Located on the racks for clean dishes there were 2 butter brushes with burned and shredding bristles. COS. Items were placed in trash can. Located in the bar area the lid on top of the ice is cracked and being held together with duct tape.
KDA_4_30111 - --The small RIC by the make table had an internal temperature of 52.5F. The PHF food items had to be VD. COS. All PHF food items were removed. Unit will be checked by repair person.
KDA_4_50111B - --The door gaskets on the WIC units and RIC units and make table units are all broken and cracked. The door handles on the WIC and white RIC is broken and very loose. The WIC temperature curtains are all broken and cracked.

The floor areas in the kitchen area, under the equipment, under the dish area, under the tables and shelving and under the cooling units have an accumulation of black debris.
KDA_7_10211 - --Located in the pot/pan dish area there was a working spray bottle with clear liquid and no label. PIC said it was degreaser. COS. Chemical was poured down sink.
KDA_7_20111B - --Located on top of the dish machine and above the clean and sanitize side of dish machine, there was one bottle of air deodorant spray. COS. Bottle was moved.
KDA_7_20212A2 - --Located in the bar area where the employee was dong dishes, the working sanitize solution of chlorine was over 200ppm which is at a concentration above the label directions. Label directions stated 100ppm. COS. Employee drained some of the solution and diluted with water to reach proper concentration.
