The pest control company concentrated on cockroaches, ants, centipedes, crickets, millipededs, silverfish and spider (routine spray-none found). There were no dead or alive bugs detected during the inspection. KDA_4_20211A2 - --On the utensil rack in the ware wash area, there were 2 tan square plastic food grade containers with broken edges. COS-discarded. (Note: 2 out of 30+ containers, less than 10%). KDA_4_70211 - --The mechanical dish machine was in use, utilizes chlorine and measured at 0ppm. COS-KDA 61 primed the machine and now it is reading 50ppm. There were air bubbles in the plastic line connecting from the machine to the sanitizer bucket. KDA_5_20212A - --The hot water was turned off at the handsink in the ware wash area. The person in charge said it still has a slow leak but it will be fixed. COS-turned hot water back on. KDA_7_10211 - --There was a white plastic bucket with clear liquid being stored directly on top of the mechanical dish machine. The bucket was not labeled and it was bleach water. KDA_3_50116A2 - --In the reach in cooler below the make table there was a metal container of corn beef hash at 46.7F, a metal container of ready to eat sausage patty at 46.5F and a metal container of shredded cheese at 56.2F. The person in charge said he turned the make table on around 8:00am and the food was moved from the walk in cooler into the unit. The food was tempted around 8:40am (less than 4 hours). COS-left the unit doors and lids closed. The ambient air temperature of the unit was reading 33.8F before 10:00am. KDA_3_50118A1 - --In the walk in cooler on a speed rack there was one pumpkin pie date marked 10/11/15 (held past 7 days) and 5 peach pies date marked 10/11/15 (held past 7 days). COS-discarded. KDA_4_20211A2 - --On the utensil rack in the ware wash area there were two (1/3) plastic food grade containers with broken edges. COS-pulled and not going to be used for food items. There was also two large plastic lids and three medium plastic lids with broken edges. COS-discarded. There was one large plastic container with a broken edge. COS-pulled and not used for food items. KDA_4_50111A - --The walk in freezer handle is broken. KDA_4_60111A - --On the utensil rack in the ware wash area there was one plastic and two metal 9.39 qt food containers with dried food debris on the inside and outside of the containers and stacked clean. There was one (1/3) plastic food grade container with dried food debris on the inside of the container and stored clean. There one was one (1/6) metal food container and two square metal food containers with dried food debris on the outside and stored clean. On the hook to the right of the 3 compartment sink there were two metal tongs with dried food debris on the food contact surfaces and stored clean. There was also one cone-shaped metal colander with dried food debris on the inside and stored clean. COS-moved all of the items to the ware wash area. KDA_4_60111C - --There is minor dust build up on the fan blowers in the walk in cooler. There is minor dust inside of the grill hood. There is grease build up on the fire compression system pipes above the stove. KDA_4_60211E4 - --There is mold build up on the inside of the Pepsi pop nozzle at the soda fountain machine. KDA_4_70211 - --The mechanical dish machine was in use, utilizes chlorine and measured at 0ppm. KDA61 and the person in charge primed the machine and still reading 0ppm. COS-the person in charge called for the machine to be service. The plastic tube was not inserted far enough within the plastic casing down in the sanitizer bucket. Now it is reading 50ppm and rewashed all of the dishes. KDA_4_90311B - --On the utensil rack there were metal containers stacked wet/clean and not able to properly air dry. COS-separated the current clean dishes to air dry properly and moved the older containers to the ware wash area. KDA_5_20212A - --The hot water was turned off at the handsink in the ware wash area. The person in charge said they had a pipe leaking but it is fixed. COS-turned hot water back on and reading 100F. KDA_5_20515B - --The hot water handle in the men's restroom is broken (not able to shut it off) and hot water is flowing from the faucet. The faucet at the handsink in the ware wash area leaks when the water is turned off. KDA_6_20214 - --The employee restroom does not have a self-closing door and it opens into the kitchen. KDA_6_30114 - --There are no handwashing signs in the men or women's restroom. KDA_6_50111 - --In the men's restroom, there is a whole in the wall above the urinal. One of the light fixtures in the women's restroom does not have a protective cover. The wall panel behind the ware wash area is peeling exposed. KDA_6_50112A - --There is grease and egg shells on the floor behind the stove. There is dust on the walls and ceiling above the steam table and make table. There is mold on the wall behind the mechanical dish machine.
