- KDA_3_50118A2 - --Located in the reach in cooler (RIC) under the make table (MT), there was a container of Italian sausage crumbles that did not have a date. Person in charge (PIC) indicated that the Italian sausage was opened and put in the RIC on 10/10. Corrected on site (COS)- dated.
- KDA_3_50118A3 - --Located in the walk in cooler (WIC), there were six containers of pizza sauce that were dated from 10/10 through 10/17. PIC verified dates. COS- educated on date marking for 7 days, redated.
- KDA_4_20311 - --Located in a cup at the prep table of the kitchen, there was a probe thermometer with a range between 120F and 200F. This was the only probe thermometer available.
- KDA_4_60111A - --Located in the cabinet under the front counter, there were 10 plastic food container lids stored as clean that had stickers or sticker residue on them. Located in the storage room, there was a stack of 6 plastic food containers stored as clean that had stickers or sticker residue on them. Located on a shelf above the three vat sink, there was a stack of 4 plastic food containers stored as clean that had stickers or sticker residue on them.

- KDA_7_10211 - --Located on a cart at the front counter, there was an unlabeled container of blue liquid with a wiping cloth in it. PIC identified this liquid as sanitizer. COS- labeled. Located on a table behind the three vat sink, there was a container of bubbly water which the person in charge identified as soap and water. COS- labeled.

- 4-302.14 - --The test strips that were available had been previously dropped in the sanitizer water and were all colored blue from the sanitizer. There were no other test strips available.
- KDA_3_30212 - --Located on the kitchen counter next to the MT, there was an unlabeled container of brown powder which the PIC identified as cinnamon sugar.
- KDA_3_30412B - --Located in the kitchen MT, there were cups with no handles in the containers of beef and sausage crumbles. Located in the WIC, there was a container of parmesan cheese with a single use cup with no handle stored in contact with the cheese.

- KDA_3_50116A1 - --Located at the pizza buffet, there was a sausage pizza at 123F and a supreme pizza at 121F. PIC indicated that they had been on the buffet for 45 minutes. These pizzas were on the outside edge of the warmer. COS- pizzas were discarded. Educated PIC on keeping pizzas directly under the warmer.
- KDA_3_50118A1 - --Located in the WIC, there was a container of chopped ham with a date of 9/20. PIC verified date. COS- discarded.
- KDA_3_50118A2 - --Located in the WIC, there were four containers of pizza sauce with no date. PIC indicated that they had made the sauce on 9/26. COS- dated.
- KDA_4_20311 - --Located in a cup on the counter next to the kitchen make table, there were two probe thermometers which read 45F and 95F. The ambient air temperature of the kitchen was 74F. These thermometers did not have a way to be calibrated and there are no other probe thermometers available.
- KDA_4_60111A - --Located on the bottom shelf on the counter with the pizza dough roller, there were three small pizza pans stored as clean that were soiled with dried food debris and flour. COS- moved to wash area. Located in the storage area, there was a stack of 9 plastic food containers and 12 plastic food container lids stored as clean that had stickers on them. 99% of the plastic food containers and plastic food container lids located in the first cabinet under the pass through counter at the front counter had stickers on them.
- KDA_4_60111C - --The outside of the pizza ovens have an accumulation of grease, dust,and dried food debris. There is food debris on the bottom shelf of the Kenmore Elite reach in freezer. There is an accumulation of dust on the vent cover of the WIC.
- KDA_4_60211E4 - --There is mold under the soda nozzles and on the ice chute of the customer self serve soda fountain at the front counter.
- KDA_4_90311A - --Located in the storage room, there was a box of single use insulated cups stored on the floor. There are pizza pans stored as clean on the bottom shelf of the counter with the pizza roller.

- KDA_6_50112A - --There is debris on the floors of the kitchen and storage room. There is food and debris on the floor of the WIC. There is flour on the bottom shelf of the counter with the pizza roller on it. The bottom shelf under the counter next to the make table has debris on it.
- KDA_7_20111A - --Located on a shelf above the mop sink, there was a box of food film plastic wrap stored next to a bottle of dish soap. There was also a box of aluminum foil and a box of single use plastic food bags stored next to a container of dish soap. COS- moved.
