Note: An employee said she was unsure when the container was opened. Corrected on Site - Discarded In the Reach in Cooler (where wraps are prepped) there was a container of prepared on site coleslaw that had a 10/28 date dot. This date dot was partially covering another date dot that had 10/27. Note: An employee said that originally the container had coleslaw from 10/27, but more coleslaw was added to it today. Corrected on Site - Educated/Re-dated

- KDA_3_30511A1 - --There were flats of (packaged) bread buns stored next to the hand washing sink. Note: There is no splash guard at the sink and when employees wash their hands the water splashes onto the packages of buns.
- KDA_3_50115B - --The leaf lettuce was cooling in the Walk in Cooler inside a tall plastic bucket with a sealed lid. The bucket of leaf lettuce used at the Make Table, was stored about 12" above the cooling line. Corrected on Site - Lids were lifted and educated employee about having the food containers inside the Make Line.
- KDA_4_20211A2 - --There were 3 cracked food trays, stored as clean on the south shelf. There was an icing spatula, stored as clean above the 3 vat sink, that had cracks along the edge. Corrected on Site - Removed
- KDA_4_50112 - --The cutting boards at the Food Prep sink have several deep scratches.
- KDA_4_60111A - --At the north storage shelf there were 5 plastic food containers (stacked) with dried sticker residue on the outer surfaces, 3 plastic food buckets (stacked) with dried sticker residue on the outer surface, 2 colanders with dried food residue build up on the food contact surface, and a whisk with dried food debris build up on the food contact surface. An employee said all items were stored as clean. Corrected on Site - All items were removed and placed at the 3 vat sink for cleaning The Reach in Cooler, below chicken breading area, has dried blood and caked on grime on the interior surfaces for doors, throughout the door seals and throughout the base. The door seal for the west Reach in Cooler (in the serving area) has grime build up.
- KDA_4_60111C - --There is build up of dust on the outside surface of the vent hood (above cooking equipment).
- KDA_6_30114 - --There are no hand washing signs posted for the hand washing sinks in the Kitchen.
- KDA_7_20111A - --There was a spray bottle of sanitizer stored next to black trays with single use papers (Kitchen storage shelves). In the Serving area cabinets, there was a container of cleaning tablets that was stored with coffee filters and single use cups. Corrected on Site - All chemicals were moved
