KDA_2_30114I - --A cook was wearing gloves when she moved the trash can with her hands, then she removed her gloves and handled money at the register. After handling money, she donned new gloves and handled bread for a sandwich without handwashing prior. COS- educated staff on proper handwashing, the cook voluntarily discarded the bread and washed her hands properly before donning new gloves.
KDA_2_40111 - --There was an employee's half eaten food and drink stored on the back prep table next to the soup kettles.
KDA_3_50116A2 - --There were packages of sliced cheese (American) 49F, swiss cheese 52F, pepperjack cheese 48F, mild yellow cheddar cheese 50F and smoked gouda 51F in the display cooler. The sliced pieces were wrapped in plastic wrap. The ambient temperature measured at 36.5F. PIC stated the swiss cheese was cut approximately 2 hours prior and the other cheese was sliced last night at approximately 8PM. COS- PIC voluntarily discarded all sliced cheeses except for the swiss which was placed in the walk in cooler.
KDA_4_20211A2 - --There were 4 dispensing metal ladles hanging on the clean rack in the back with large cracks and rust along the food contact surfaces. COS- PIC voluntarily discarded.
KDA_4_204112B - --The only internal thermometer available for the display case was broken and reading 55F. The ambient temperature measured at 36.5F.
KDA_4_50111A - --There is a Beverage Air make table in the back that is currently broken and not in use. PIC stated the item is going to be removed.
KDA_4_60111A - --There were approximately 10-15 plastic food storage containers with old date sticker residue along the sides being stored on the clean utensil rack in the back. Not corrected on site. There was old food residue build up on the food contact blade of the commercial opener in the back. COS- PIC moved the opener to the warewashing area for additional cleaning.
KDA_4_60111C - --There are several clean metal containers stored on the bottom shelf in the back room. The shelf is covered in dirt and rust and is not an easily cleanable surface.
KDA_4_60211E4 - --There was old powder residue build in the inside dispensing tubes of the cappuccino unit in the lobby.
KDA_4_70211 - --There was no Chlorine reading in the dishmachine while in use by staff. PIC changed Chlorine container out but there was still no chemical reading. Recommended setting up 3 vat to clean dishes.
KDA_5_20515B - --There is a steady leak coming from the cold handle of the faucet on the 3 vat sink in the back.
KDA_6_20215A3 - --There is 1/4 inch gap under the front entry door large enough to allow the entry of pests. The back exit door was propped open a half inch which is large enough to allow the entry of pests.
KDA_6_30111 - --There was no handsoap available at the handsink in the warewashing area. Not corrected on site.
KDA_6_30112 - --There were no disposable towels at the handsink in the warewashing area. Not corrected on site.
KDA_6_50111 - --There were several cracked and broken floor tiles missing in front of the handsink in the back, under the 3 vat sink in the back and under the bulk ice unit.
KDA_6_50114A - --There is dust build up along the inside of the hood above the ovens in the back prep area.
KDA_6_50116 - --There was a wet mop stored in the mop bucket not allowed to properly air dry.
KDA_7_20111A - --There was a working container of oxygen orange cleaner stored directly next to 50lb bags of flour on a rack in the prep area. Discussed moving the chemical to a different area with the PIC. Not corrected on site.
KDA_7_20111B - --There was a working container of oxygen orange cleaner stored directly above 50lb bags of flour on a rack in the prep area. There was a working spray bottle of glass cleaner stored directly on top of a box of cappuccino mix under the front counter. COS- PIC moved the chemical to a different area.
KDA_Defacto - --A cook handled money at the register and then donned new gloves without handwashing prior. Inspector intervened prior to any food handling.
