KDA_3_30412D - --There were approximately 10 metal spoons stored in the dipping well at the front without the water running. The spoons were in use. COS- PIC turned water on.
KDA_3_30511A1 - --There was a box of chocolate mix in a cabinet at the front directly below the pipes of the dump sink. There is no protective shielding under the piping in case of leaks.
KDA_3_50118A2 - --There was an opened gallon of milk in the walk in cooler without any date marking. PIC stated the milk was opened Monday 9/14. COS- PIC correctly date marked. This was the only date marking issue in the establishment.
KDA_4_60111A - --There was a bucket holding approximately 15 clean metal spoons with a 1/2 inch of dirty standing water in the bottom. COS- PIC moved the container and the spoons to the ware washing area for additional cleaning.
KDA_4_90311A - --There was a box of single service spoons in a cabinet at the front directly below the pipes of the dump sink. There is no protective shielding under the piping in case of leaks.
KDA_5_20213 - --The spray hose at the 3 vat sink was hanging below the flood rim level by approximately 3 inches creating no air gap for backflow prevention. COS- PIC adjusted the hose so that it hangs above the flood rim level by at least twice the diameter.
KDA_8_30411A - --The old license (4/1/2014-3/31/2015) is hanging in the facility. PIC called owner who has the new license at personal residence.
KDA_Defacto - --All potentially hazardous foods in the freezers are frozen. --milk 40 (walk in cooler) shake mix 39 (reach in cooler front) --QT measured at 200ppm in 3 vat sink, QT test strips provided. --There is a pest control company contracted, no paperwork provided. No activity.
