KDA_3_30412E - --The metal ice scoop was being stored on top of the ice machine (not a sanitary surface). COS-placed ice scoop in the ice machine handle up.
KDA_5_20515B - --The handsink located at the back bar leaks below when water is supplied.
KDA_6_30114 - --There were no handwashing signs in the men or women's restrooms. COS-left 2 handwashing stickers.
KDA_6_50111 - --The toilet seat in the left stall in the women's restroom is missing.
KDA_6_501114A - --In the back storage room there is equipment and miscellaneous items not in use. An employee said it has been a slow process cleaning it out but the PIC is aware of it.
KDA_6_50112A - --At the front bar there ware beer bottle caps and trash on the floor. (Note: cleaned daily).
KDA_7_20111B - --There was a working squirt bottle of hand sanitizer and a bottle of rubbing alcohol being stored on top of the ice machine. There were 2 spray cans of Quick-Fast Drying All Purpose Spray Enamel being stored on top of the hot water tank. COS-removed all chemicals.
