  • KDA_4_20211A2 - --On a utensil rack there was one Sterilite plastic container that was cracked on the bottom and it is used to store food items. The food items are wrapped in foil before placing them in the container. COS-discarded.
  • KDA_4_60111A - --On the utensil rack there was one plastic Sterilite container with a sticker left of the outside and stacked clean. COS-moved to the ware wash area. On another utensil rack there was one (1/3) plastic food grade container with a sticker left on the outside and stored clean. COS-moved to the ware wash area.
  • KDA_5_20515B - --The hot water handle on the handsink located to the right of the make table is broken. It does not supply hot water, the employees are using the handsink that is located in the ware wash area just on the other side of the wall (less than a foot away).
  • KDA_7_20111B - --In the employee restroom there were two working spray bottles of Kitchen Equipment Cleaner and Glass Cleaner being stored directly on top of the paper towel dispenser. COS-removed chemicals.
  • KDA_Defacto - --Have Quaternary Ammonia test strips. --In the walk in cooler there was a large round plastic food grade container of pizza sauce at 64.6F. It was made today around 1:00pm and tempted around 2:45pm. --Make Table tomatoes 37.7F ground sausage 40.5F noodles 40.2F shredded provolone cheese 41.1F RIC (below MT) cubed ready to eat chicken 42.4F pepperoni slices 41.4F blue cheese dressing 39.8F Pizza sauce on counter 41.6F Oedesa RIC boneless chicken wings 39.1F shredded provolone cheese 41.4F WIC noodles 37.1F Italian sausage 38.5F --The 3 compartment sink was set up, utilizes Quaternary Ammonia and measured at 300ppm.
