KDA_4_60111A - --On a shelf below the large metal prep table there was 2 metal scoops with dried food debris on the food contact surface and stored clean. COS-moved to the ware wash area.
KDA_5_20515B - --At the mop sink there was a shut off valve below the atmospheric vacuum breaker. One hose was attached to the chemical dispenser which is installed with a backflow preventive device and the other hose was hung up above the flood rim which provides an air gap. The atmospheric vacuum breaker does not leak when water is supplied. KDA 61 educated to the person in charge about the constant pressure from the shut off valves on the atmospheric vacuum breaker and discussed placing a Vented T-Valve.
KDA_6_50111 - --There are broken floor tiles below the Beverage-Air reach in freezer.
KDA_7_20111A - --On a metal prep table there was a grey sanitizer bucket being stored next to a box of individual packets of Mayo and single use plastic lids. COS-removed the sanitizer bucket.
KDA_7_20111B - --In the ware wash area there was a working spray can of Chewing Gum & Candle Wax Remover and one working spray can of Stainless Steel Water Based Cleaner/Polish Aerosol being stored directly on top of the paper towel dispenser. COS-removed the chemicals.
