KDA_3_20215 - --On the can rack there was one can of Pineapple Tidbits with a deep dent on the side seam. COS-pulled for vendor credit.
KDA_3_30412E - --The metal ice scoop was being stored directly on top of the ice machine (this is not a sanitary surface). COS-removed the scoop off of the ice machine and placed in the plastic ice bucket.
KDA_3_50116A2 - --On a metal prep table there was an 18qt plastic food grade container of pizza sauce at 50.1F. The person in charge said it has been sitting out since 11:00am and was tempted around 3:00pm (right at 4 hours). COS-discarded. (Note: discussed the sauce being pH tested, more documentation is needed at this time).
KDA_3_50118A2 - --In the walk in cooler there was a plastic tub of individual cups of marinara sauce (made on site) without a date. The person in charge said they were made on 10/13/15. COS-item was date marked.
KDA_4_20211A2 - --In make table 3 there was a small plastic food grade container that had a broken edge and being used to store diced tomatoes. COS-placed the tomatoes in a new container and discarded the broken container. In a plastic divider on the clean dish side, there was one rubber spatula with a deep cut on the food contact surface side (not easy to clean). COS-discarded.
KDA_4_60111A - --On a metal prep table the large meat slicer had oil residue left on the blade. There was also a metal square produce slicer and another metal produce slicer with dried food debris in the blades. The person in charge said they were used yesterday. COS-moved to the ware wash area to be cleaned. Below the metal prep table there was a container with 3 large white round plastic lids, 10 clear round plastic lids and 8 small clear plastic lids with sticker residue and stored clean (can hold bacteria). COS-moved to the ware wash area. On top of the ice machine there was an 18qt plastic food grade container being used as an ice bucket with a sticker left of the side of the container (can hold bacteria). COS-moved to the ware wash area. On the utensil rack there was one plastic sauce bottle with a sticker left on the outside and stored clean. COS-moved to the ware wash area.
KDA_4_60111C - --There is dust/flour dust build up on the inside of the pizza oven hood.
KDA_4_60211E4 - --There is minor mold build up in the right corner on the plastic ice deflector inside of the ice machine.
KDA_6_20215A3 - --At the back exit door there is an 1/8" gap at the top left corner. This could allow entrance for pests.
KDA_6_30311A - --There is one light bulb burnt out in the walk in cooler.
KDA_7_10211 - --On the floor there was a grey plastic tub with clear liquid and was not labeled. The person in charge said it was soap and water. COS-labeled tub.
KDA_7_20111A - --There was a working spray bottle of Oven, Grill & Fryer cleaner being stored directly next to clean metal pizza pans. COS-removed chemical.(Note: no leakage detected).
