- KDA_4_30112A - --In the meat cutting room they do not have a three compartment sink. They have applied for a variance. They said they faxed it on 7/31/15.
- KDA_4_60111C - --In the deli walk in cooler the fan covers have an accumulation of dust. COS-The covers were taken off and cleaned while inspector was there.

- KDA_6_50111 - --The walk in freezer on the west wall has an accumulation of ice on the floor. Two of the fans are not running. One fan cover is hanging off the fan blades. There is an accumulation of ice hanging off the fan unit. Owner said they did not repair because they are getting a new unit.In the cake baking area, there is a stain on the ceiling over the Baxter oven. In the women's rest room there is stained ceiling tile.
- KDA_6_501114A - --Outside on the south side of the building, there is an accumulation of wire racks and debris.
- KDA_6_50112A - --In the cake baking area next to the Baxter oven there are cardboard boxes holding single serve items that are stacked to the ceiling.
- KDA_7_20111A - --In the back room where they use the green plastic wrap to wrap the food, the nozzle of a bottle of sparkle glass cleaner was pointing toward a roll of green wrap. COS-Employee moved the bottle. This is a new violation.
- KDA_Defacto - --In the hot holding case: gravy 165.8F, mixed vegetables 142.3F, pasta 142F, chicken tender 160F, chicken livers 155F, shrimp 149.1F, burrito 155F, chicken breast 154.5F.
