COS-date marked the item.

  • KDA_4_60111A - --On a lower shelf and in a box, there were 3 round plastic food grade lids with labels left on them and stored clean (can hold bacteria). COS-discarded. On a lower shelf there were 2 plastic food grade containers with labels left on them and stacked clean (can hold bacteria). COS-moved to the ware wash area. The large mixer had dried food debris on the metal frame where the whisk is inserted. The large meat slicer also had dried food debris on the blade. The kitchen manager said he does not use them. COS-cleaned both.

  • KDA_3_50118A2 - --There was a open package of commercially processed ham sliced in the establishment that had no use by date on the package. PIC said it was cut on 10/10/15. COS Item was date marked.
  • KDA_4_20211A2 - --There was a plastic food grade bowl stored as clean on shelf in the kitchen area were the plastic was flaking on the inside of the bowl. COS item was discarded.
  • KDA_4_204112B - --There is no thermometer in the RIC-3(frigidaire)to measure internal temperature of PHF'S.
  • KDA_4_60111A - --There was a plate stored as clean on a shelf in the kitchen area that had dried food debris on the plate from a previous use. COS Item was cleaned.
