KDA_2_40211A - --The store manager hair was pulled back into a ponytail. Her hair was wrapped around on her shoulder towards the front of her body. COS Informed PIC that hair needs to be pulled in the back and restrained so that is not potentially exposed to foo clean equipment, utensils, linens, or single use articles. PIC went to bathroom and corrected hair style.

KDA_3_30212 - --A bottle of clear liquid was siting on the front counter did not have a label. PIC identified it as water. A bottle of yellow liquid did not have a label on the bottle. PIC identified the liquid as butter. COS Label was put on bottle.
KDA_3_50119B2 - --Butter that was in a squeeze bottle was sitting on top of the microwave without a marked time. PIC said it had been there since 8am. COS A time stamp was put on the bottle to be discarded in 4 hours.
KDA_4_20211A2 - --Cracked food contact containers were stacked and stored on a clean storage rack. The cracks were in the interior surface. COS Containers were discarded.
KDA_4_60111A - --Observed a container with dried food debris on the exterior surface. COS The container was removed and placed in the warewashing area to be washed, rinsed, and sanitized.
KDA_4_90111A - --Plastic food contact containers were stacked wet on a clean storage rack. COS Containers were removed and placed in the warewashing area to be washed, rinsed, and sanitized.
KDA_5_20511B - --Observed a pipe scrubber in the handwashing sink that is close to the front counter. COS Scrubber was removed.
