KDA_3_50114A1 - --6 small pans of Cooked rice were cooling on the counter at a temperature of 112F. PIC said rice had been there for one hour. COS. Rice was spread onto flat pans and pan was placed in freezer to get rice to a temperature of 70F in 1 hour. Educated staff.
KDA_4_50111B - --The handle on the RIF door is broken and cracked.
KDA_4_60111C - --The RIF interior has an accumulation of food debris on the bottom shelving. COS. RIF was cleaned.
