4-302.14 - --REPEAT: The owner uses bleach as a sanitizer on her dishes. She does not currently have bleach test strips. She has spoken with her supplier about them since her previous inspection and is waiting for them to arrive.
KDA_3_50118A2 - --REPEAT: In the cooler in the kitchen there is a tupperware container of baked beans that was kept after being previously served. There is no date on the container. The owner said she did the baked beans this past Monday (11/30/15). Corrected on Site (COS) - she dated the container. In the cooler in the kitchen there is a container of cooked beef tips with no date marking. The owner said she did the beef tips last Friday (11/27/15). COS - she dated the container.
KDA_4_90311B - --REPEAT: There are non-inverted and uncovered plates sitting on top of the microwave.
KDA_5_501113 - --REPEAT: There is no lid on the outside trashcan. The lid it once had was broken. The owner is looking into a trashcan with a lid.
KDA_6_10111A - --REPEAT: The floor behind the bar where all the ware washing and food prep is done has a thick carpet/rug in place.
KDA_6_20214 - --REPEAT: The restroom doors have springs attached to make them self-closing, but the springs are currently detached from the door frame so that the doors will not self-close.
KDA_6_20215A3 - --REPEAT: The front door has a narrow 1/4 to 1/2 inch gap at the bottom corner of the threshold. The side door has a narrow 1/8 to 1/4 gap at the bottom threshold. The side door is not self-closing.
KDA_6_50119 - --REPEAT: The restroom doors are open during the inspection. Cleaning and maintenance are not being performed.
KDA_7_10211 - --REPEAT: There are unlabeled containers at the 3 vat sink and hand sinks in the kitchen and restrooms. These were identified as dish soap and hand soaps by the owner. Reminded her that they need to be labeled. COS - she labeled these items.
KDA_7_20212A2 - --REPEAT: There is a Hot Shot Wasp & Hornet Spray (EPA Reg 9688-190-78845) in the bar area. Reminded the owner that on the previous inspection when she was asked if this and a fly spray were used inside, she was told this needed to go home with her as it cannot be used in the food establishment. COS - She took it to her car to remove it from the establishment. She asked if it could be used safely outside. Let her know that it is fine for the spray to be used outside only.

Let her know these are expired and for a different sanitizer type than what she is currently using. Explaining the 1-2-3 method (1 cap of bleach to 3 gallons of water) for measuring bleach.
3-501.17(A) - --There is a container on the counter that says "Pork Chili 6/11" on it. The owner said that is marking from when someone wrote on it with permanent marker. She said that it is actually Ham & Beans which she prepared on 11/4. She froze it 11/5. She pulled it from the freezer today (11/18) to thaw. COS - going over date marking with freezer time involved with her. She is dating this appropriately.
4-102.11(B)(1) - --There is a plastic "Thank You" grocery bag in the drawer in the cooler. There are tomatoes stored inside of this bag in direct contact with the plastic of the bag. Letting the owner know that the tomatoes may not be stored in this bag.
KDA_2_30115 - --The owner washed her hands at the 3 vat sink. Corrected on Site (COS) - explained to her that this is a violation. She will use the hand sink in the future.
KDA_2_40111 - --There is an open can of NOS in the cooler on the top shelf above food items. The owner said this was her personal drink.
KDA_3_10111 - --There is an open half gallon of milk in the cooler with no date marking on the bottle. COS - The owner did not identify when this milk was opened. When asked, she said it needed to be thrown away and poured it out.
KDA_3_30211A1a - --There is raw pork bacon on the top shelf of the cooler. This is directly above raw lettuce and tomatoes in the drawers below. Let the owner know this needed to be rearranged.

They are as follows: Hot Links - opened Monday (11/16) per the owner Baked Beans - prepared Monday (11/16) per the owner Sliced Tomato - prepared Monday (11/16) per the owner Sliced Lettuce - Prepared Monday (11/16) per the owner COS - she is dating all of these items appropriately. In the walk-in-cooler there is a container of beef stew that is not dated. The owner said she prepared this Monday (11/16). COS - she is taking this home for her husband to eat.
KDA_4_204112B - --There is no thermometer available in the cooler in the kitchen.
KDA_4_60111A - --There is a plastic container storing corn meal in the back storage room. There is sticker residue on the outside of the container. The owner said this was left over from when she originally purchased the container and she didn't get it scraped off. Let her know that this needed to be removed from the container during the cleaning process.
KDA_4_60111C - --There is slight dust accumulation on the blades of the fan in the bar/prep area.

Let the owner know she should look into other bleaches that are approved for use on all food contact surfaces. --There is cooked pork and cooked meatballs in sauce in the cooler in the kitchen. The owner identified these both as having been prepared yesterday in the afternoon/evening time. --There is no mop sink - see business notes. --Updated license rating as owner is cooking raw meats and keeping left overs for daily lunch specials. --Walk-in-cooler: Beef Stew - 43.7 F Cooler (Prep Area): Milk - 41.7 F There are green beans sitting out on the counter at 59.2 F. These were just removed from a can (in the past 10 minutes, per the owner) to be heated up in the microwave.
