Located on a prep cart next to the make table, there were two unlabeled bottles of brown liquid, an unlabeled bottle of clear liquid, and an unlabeled bottle of light brown liquid which the PIC identified as soy sauce, teriyaki sauce, vodka, and oil. COS- labeled. Located on a shelf next to the clean plates, there was an unlabeled container of brown liquid which the PIC identified as teriyaki sauce. COS- labeled.
- KDA_3_30412B - --Located at the front of the kitchen, there were two large bins of rice, both of which had bowls with no handles stored in contact with the rice. COS- removed bowls.

- KDA_4_60111C - --There is an accumulation of grease and dried food debris on the kitchen grill, on the front of the microwaves, on the crockpots, and on the stove in the kitchen. There is an accumulation of grease and dried food debris on the front of the microwave at the sushi station.
- KDA_4_60211E4 - --There is mold on the inside rim of the ice machine. There is mold on the ice chute and under the soda nozzles at the wait station soda fountain. There is mold in the soda nozzle at the bar area.
