  • KDA_6_20211A - --WIC light bulb is not shielded or otherwise protected.
  • KDA_6_30114 - --Men's toilet room hand washing sink did not have a sign or poster that notifies food employees to wash their hands. COS by providing hand wash reminder stickers.
  • KDA_7_10211 - --Blue spray bottle with liquid inside near the front counter was not labeled. PIC said it was windex and COS by labeling. Blue spray bottle with liquid inside near the slicer was not labeled. PIC said that it was mineral spirits and COS by labeling. Four squirt bottles on the 3 Vat sink containing liquids were not labeled. PIC identified the blue as Dawn Soap, the clear as bleach, the dark yellow as pine sol and the light yellow as vinegar mixture. PIC COS by labeling correctly.
  • KDA_Defacto - --Complaint Inspection was based on report about the wire shelving that is rusted and has been covered with wall paper. One area of the shelving that was above food was observed to have rust and had been covered with wall paper. There was no evidence of contamination but replacement of the shelf would prevent any possibility of having physical contamination in the ice well below the shelf. --On Ice: Shredded Cheese 41, 39
