- 4-302.14 - --The manager provided chlorine test strips for their chlorine tablets, but they are missing the color coded sheet for verifying concentration. She said that an employee mistakenly dropped the original box into the water and they lost the color coded sheet/most of the test strips. Let her know she needed to see if they could get a replacement sheet in to verify concentration stays between 50ppm to 100ppm.
- KDA_4_10111A - --There is a gray sterilite bucket sitting on top of the ice machine that is used as the ice bucket. There are no markings on this bucket indicating that it is a food grade bucket. The ice is put directly into the bucket when it is used. Let the manager know she needed a food grade bucket, or documentation from the manufacturer that this bucket is food grade.
- KDA_4_60111A - --The containers holding the packets of creamer, sugar, salt, and pepper have stickers on the outside of them. These stickers were identified as remaining from the original purchasing of the container. Let the manager know that the containers need to have the stickers cleaned off.
- KDA_4_60111C - --There is an accumulation of dust on the fan covers in the walk-in-cooler.
- KDA_4_90311C - --At the self-service table in the bar/dining area there are Styrofoam plates and to-go containers that are outside of their original protective packaging, neither covered nor inverted.
- KDA_4_90411B - --There is a tray of plastic silverware for self-service on the counter in the dining/bar area. The handles of the knives, forks, and spoons are facing different directions, making it so that touching one handle may result in touching the food contact/lip contact surface of another utensil.

- KDA_5_20315 - --The carbonator has a dual check valve with no vent on it. Let the manager know that the soda pop company needs to come in and install a dual check valve with an intermediate vent downstream of the screen/any copper in the line, but upstream of the carbonator.
- KDA_5_50117 - --There is no trash can with a lid available in the women's restroom.
- KDA_6_30114 - --There are no hand washing signs posted at the hand sinks. Corrected on Site (COS) - provided with signs to be posted.
- KDA_6_50112A - --There is slight grime buildup in the corner behind the ice machine.
- KDA_7_20111A - --In the cabinet under the hand sink in the bar area there is Window Cleaner being stored directly next to straws and napkins. Let the manager know this needed to be rearranged.
- KDA_7_20111B - --In the kitchen there is charcoal lighter fluid being stored inside of a clean stainless steel pan. It is also directly above a cooler stored below the table. COS - the manager moved the lighter fluid.
- KDA_Defacto - --Kitchen Cooler: Hot Dogs - 40.1 F --Ownership verified. --There are non-inverted and uncovered dishes throughout the kitchen. The manager said that they never use these and the only time they are used is when groups take them to the veteran's home and cook meals for them prior to bringing the equipment back to this kitchen. She said the only thing they do with their kitchen is warm hot dogs on the weekends in their hot dog warmers. --There are two spray cans of hot shot bugger killer that is not labeled for use inside of a food establishment. The manager said she has been here for two years and has never seen it used. They have a pest control company coming in routinely for pest management. --This establishment heats commercially prepared hot dogs for customers, mainly just on the weekends.
