3-302.11(A)(2) - --In the WIC there was an open carton of raw hard shell eggs being stored directly above closed boxes of raw beef. COS-placed a plastic try as a second barrier. (Note: no leakage detected from the eggs). In the Kenmore Refrigerator there was a box of hard shell eggs being stored directly on top of a closed box of raw bacon. COS-rearranged items. (Note: no leakage detected).
KDA_3_30412B - --In the flour and sugar containers there were metal scoops in direct contact with the products. COS-the person in charge removed the scoops.
KDA_3_30412E - --The metal ice scoop was sitting on the plastic surface on top of the ice machine (not a clean/sanitary surface).
KDA_3_50116A2 - --There was a package of cheese sitting out on the counter next to the grill at 58.7F. The person in charge said it has been out since 7:00am and it was tempted around 8:25am (less than 4 hours). COS-placed in the WIC. In a metal container next to the grill there was partially cooked bacon at 46.3F. It was placed out around 7:00am and tempted around 8:30am (less than 4 hours). COS-placed in the WIC. In the WIC there was a plastic food grade container of sliced ham at 58.9F. It was in another RIC over night and the cooler was not working properly (froze over). COS-discarded.
KDA_4_20211A2 - --In a plastic container next to the microwave there were 5 rubber spatulas with cracked edges and deep cracks on the food contact surface side. COS-discarded 2 and trimmed 3. In a utensil drawer in the ware wash area there was one rubber spatula with cracked edges. COS-trimmed. In the ware wash area on a shelf there were two (1/6) plastic food grade containers with broken edges and a crack on the side of the container. There was also one 1.6qt plastic food grade container with broken edge. COS-discarded.
KDA_4_30111 - --RIC in the grill area was not working properly (froze over). The person in charge had to unplug the unit. COS-discarded all potentially hazardous food from the unit and plugged the unit back in.
KDA_4_60111A - --In the ware wash area on a shelf there was one metal food grade container with dried food debris on the inside and stored clean. There was also a plastic food grade container with oil residue left on the outside and stored clean. COS-moved to the ware wash area.
KDA_4_60111C - --There is grease build up on the ceiling vents above the grill. There is dust build up on the ceiling vents in the food prep area and ware wash area.
KDA_4_60211E4 - --There is minor mold build up on the pop nozzles at the soda fountain machine in the drive-thru. There is pink slim behind the plastic ice chute on the large Pepsi soda fountain machine. There is dried food debris on the plastic dispensers inside of the Hot Chocolate machine.
KDA_4_90311A - --Down in the basement there was one box of single use napkins being stored directly on the floor. COS-placed on a palate.
KDA_4_90311B - --In the ware wash area there were 14 metal food grade containers facing up and not covered. There were also 10 metal sheet pans facing up and not covered. COS-the person in charge inverted them.
KDA_6_20211A - --The light fixture in the dry food storage/utensil storage area does not have a protective cover. (Note: not sure if the light is shatter proof).
