- Toilet facilities inadequate, inconvenient, dirty, in disrepair, toilet paper missing, not self-closing doors, missing hand wash signs
- Lighting and ventilation inadequate, fixtures not shielded, dirty ventilation hoods, ductwork, filters, exhaust fans
- Critical: Potentially hazardous foods are not stored under refrigeration except during necessary preparation or approved precooling procedures (room temperature storage).

- Critical: Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.
- Miscellaneous, Economic Violation, Choking Poster, Training.
- Tobacco is used; eating, drinking in food preparation, dishwashing food storage areas
- Food not protected during storage, preparation, display, transportation and service, from potential sources of contamination (e.g., food uncovered, mislabeled, stored on floor, missing or inadequate sneeze guards, food containers double stacked)

01/15/2016 | Inspection | Comments only: 1. As specified in the Administrative Tribunal docket # 20150169, one employee per shift must be certified in food safety. A copy of a certificates of completion for each employee who completes and passes a food safety course must be submitted to the Glens Falls District Office as soon as possible. 2. There are no more cold and hot buffet stations at the facility. Facility now is order only, no more buffet.
Must correct all violations no later than 1/31/16. Must send Corey DiLorenzo an email to corey.dilorenzo@health.ny.gov addressing corrective actions taken to all cited violations no later than 1/31/16. |
