Hearing date May 12, 2016. Waiver of formal hearing issued at this time. Unless waiver is received no later than 10 days prior to hearing date, owner is to appear at 25 Moore Ave, Mt. Kisco, NY on hearing date. Non-functioning email (bjf......) deleted from system. Gloves available. All red violations and all other blue violations observed on 1/11/16, 1/25/16, 2/19/16, 2/23/16, and 2/26/16 found corrected. If during next regular inspection in the fall, violations observed today or on those dates are found recurring, notice of hearing will again be issued.

Blue item reinspection on or about 3/4/16 for blue violations observed on 2/23/16, 2/19/16, 1/25/16, and 1/11/16. Be sure end caps are provided for light tubes. All violations discussed with operator h flores. While at site, operator called J Fulgum. This writer spoke with J Fulgum. J Fulgum stated that this writer is ""busting [their] chops."" This writer explained that she is doing her job. This writer explained that there have been many repeat violations and stressed the importance of correcting these violations.
