Was allowed to be placed back in cooler due to a less than 2 hour time frame-corrected note: Ambient air temp. of cooler was 39dF 11D- Outside of mini Kenmore freezer and cooler on cook line dirty. side of fryer found with grease build-up. 15A- Walk-in floor under shelves, shelves and wall dirty. Outside of walk-in also dirty. Floor in dry storage dirty. cobwebs found on ceiling of dry storage. 11D- Switch panels dirty. Floor tiles missing under 3-bay sink and grill table.-Repeat 15B- Fan grates in walk-in with dust build up. Light fixture in kitchen above 3-bay sink not working and not shielded. 14B- Screen torn on door in back dry storage room. Gaps above and below door as well.

Ham in unit less than 2 hrs allowed to move to different cooler and corrected. Recommend keeping Phfs in bottom of unit or adjust unit to lower temps especially since unit is across from grill and lid is kept open. 8a- some foods stored on floor in walk in cooler

Also, ham in unit found to be 60dF both in unit more than 2 hours and both were voluntarily discarded by owner and corrected. 12c- faucet leaks at 3- bay sink also faucets in both restrooms also leak. 8c- clean silverware found improperly stored with food contact surfaces facing up. corrected
