- Duties of the Person in Charge (46.102)
Duties of the person-in-charge are not being followed as evidenced by the following violations.
- Eating, Drinking or Using Tobacco (46.151(a))
An open employee's beverage container was observed in on the shelf above the cook line bain marie. The container was removed.
- Hair Restraints (46.152)
Some food employees observed in the kitchen and prep areas, not wearing proper hair restraints, such as nets, hats, or beard covers. Hats were donned.
- Preventing Cross-Contamination of Packaged and Unpackaged Food by Separation, Packaging & Segregation (46.281)
Raw shell eggs stored above bread, tomatoes, etc. in the walkin cooler. The eggs were placed on the bottom shelf.
- Labeling (46.422)
Bulk and small food storage containers are not labeled as to content. The containers were labeled.
- Design & Functionality of Temperature Measuring Devices (46.590)
Thermometer broken in the small freezer at the end of the cook line.
- Sanitizing Solution Testing Device Provided & Used (46.634 & 46.674(e))
Chlorine test papers to monitor sanitizer concentration are not available.
- Frequency of Cleaning Equipment Food Contact Surfaces and Utensils (46.712)
Old food residue on the potato cutter on the pole adjacent to the prep table in the basement. The cutter was disassembled for cleaning. Old food residue on the meat slicer blade, frame and housing. The slicer was disassembled for cleaning. *Scum on the divider and walls in the ice machine. As per the chef, the ice machine will be emptied and cleaned within two days.

Old food residue on the frame of the Kitchen Aid mixer . The frame was cleaned.
- Outer Openings Protected, Exterior Walls & Roofs Protective Barrier (46.922(e)(f))
Small holes noted along the floor-wall junction behind the ice machine in the basement.
- Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
Green bait pellets scattered on the floor under the basement steps. Rodenticide blocks behind the water heater and under and behind storage racks. Rodent droppings (TNTC) in the pellets, along the floor-wall junctions in the basement and in the corner between the ice machine and the syrup rack. Dead mice on the floor under the basement steps and under the syrup rack. The chef disposed of the carcasses and stated that he will call the extermination co. to clean out the bait and droppings.
