    Observed in-use knives and /or cleavers stored between table edges or between tables, an area not easily cleanable & sanitized. COS
  • Objective of Cleaning Food Contact Surfaces & Utensils (46.711(a))
    Found lip of cutting board with heavy food debris in crack surrounding board, a food contact surface, not clean to sight and touch. COS.
  • Objective of Cleaning Grease & Soil Accumulations & Nonfood Contact Surfaces (46.711(b)(c))
    Observed the following equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces: tables including legs, refg. including refg. that is broken and used for dry goods storage, outside doors of two door freezer including front vents, door handle and door and frame for dry goods room, conv. oven, pipes, outside and inside of refgs, including Soda Refg. in dining area, shelves, outside of rice cookers, all equipment in the kitchen. Correct by 9/12/2011.
  • Materials, Design, Construction & Installation of Storage Areas & Waste Receptacles (46.881)
    Facility has front door open, allowing insect to enter, lacks tight fitting front door, Keep door closed until tight fitting door is in place, Correct by 9/12/2011.
  • Cleanability of Floors, Walls, & Ceilings (46.921)
    Floor in entrance way in restaurant missing some titles and is cracked / roughened / and is not a smooth, easily cleanable surface. 5 days to correct.
  • Outer Openings Protected, Exterior Walls & Roofs Protective Barrier (46.922(e)(f))
    Front door to the outside area of the food facility is not in the closed postion, allowing insects to enter. Tight fitting screen door is required if you want to allow air from front door into this faciltiy.
  • Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
    Food facility inspection indicates evidence of several live insect activity, (such as gnats/fruit flies/drain flies, and house flies,), in is facility, but facility does have a pest control program, Slippery Rock Pest Conrol last visit 9/2/11. Also remove died insect on window seal near freezer. Recommendation clean facility completely , have a routine schedule to eliminate the insect problem. Correct by 9/12/2011.

    Several containers of food in the walk-in cooler and two door freezer area stored open with no covering. COS

    Several food items (meat with rice, noodles, vetg), was held at 120 °F, in the large hot hold unit, rather than 135°F or above as required. SOC, PIC stated unit takes longer to heat up, They replace with new food with in one hour of initial items placed in unit.
  • Time as a Public Health Control for Potentially Hazardous Foods (46.385(c))
    Time in lieu of temperature being used as a control for potentially hazardous foods, but no reocrd of such was observed foods observed for foods thrown after 4 hours. .
  • Equipment in Good Repair & Proper Adjustment (46.671)
    Found more than 1/2 inch of ice in reach in feezer located in food storage room.
  • Frequency of Cleaning Equipment Food Contact Surfaces and Utensils (46.712)
    Found Pepsi machine noozles with heavy accum. of old dried on soda residue, noozles have not been removed for some time, can not be removed during this inspection. Correct by contacing Pepsi, replace with new noozles, or new machine, ASAP. Educated PIC on correct cleaning procesdures.

    Non-food contact surfaces, (as follows: floor under equipment, walls, equipment, shelves, Pepsi refgs., under, inside cabinet in hot hold unit, equipment under hood, both rest rooms, completely clean inside of walk-in cooler including walls, floor, ceiling &fan guards and unit, two white freezers in food storage room), not cleaned at a frequency to preclude accumulation of dirt and soil.

    Found in freezer a large cardboard box full of cream cheese wontons, placed in clean plastic food grade container, found several more than 10 containers of food in walk-in cooler uncovered, also using thank you bags for food storage in freezer, replace with commerical food grade plastic freezer bags or containers, 5 days to correct.
  • In-Use Utensils & Between Use Storage (46.302)
    Knives and / or cleavers stored between table edges which are not easily cleaned and sanitized. Common bowls or food containers observed without handles in direct contact with bulk food ingredients, (such as salt, sugar and cooked rice). Also found clean self serve utensils such are spoons and fork not stored with handles up.
  • Cooling Potentially Hazardous Food (46.384(a)(b))
    Dumpling food, was 83 degrees F out on mobile rack in kitchen cooling, instead of in walk-in cooler, PIC stated it was just cooked and placed there for about 10 minutes. Also found chicken sauce on prep-counter in 5-6 inch deep container at 120F, has been on counter about 40 minutes PIC stated, placed in 3 inch shallow pan and placed in walk-in cooler.
  • Time as a Public Health Control for Potentially Hazardous Foods (46.385(c))
    Time in lieu of temperature being used as a control for potentially hazardous foods, but cold holding unit (cut watermelon & canned fruits such as pineapple and mixed fruits), observed being held at 60 degrees F, and smaller hot hold unit held pizza, stuffed mushrroms, potatoes, mixed vetg. ... held below 135 for more than 4 hours, possible, due to no written record of food begin thrown out/discarded after 4 hour within 41 to 135 F degrees. Have a written log of foods that are not held at safe temps. (41to 135F), on your cold holding and hot holding buffet. 1 day to correct.

    Found broken and duck taped drain for ice machine located on back side of machine, pan on floor below it to caugh water, 5 days to correct.

    Found ice cream freezer with heavy ice cream spills in it. Found equipment under hood with an accumulation of old grease and food debris, 3 days to correct and defrost & clean freezer.
  • Repair & Cleaning Various Premises, Structures, Attachments & Fixtures (46.981(a)-(c)(g)(n))
    Found kitchen walls and outside walls of walk-n cooler and wooden walls in dining room & both male and female rest room walls, doors & floor behind toliets, floor under equipment in kitchen and under buffet line and ice machine, with an accum. of dirt and old food debris on them. 5 days to correct.

    Potential rodent harborage areas on the exterior of the building perimeter observed in front left area due to old kitchen equipment (large stainless box, fryers, tire, old green hose, blown trash, empty 5 gallon containers. Also found several ( more than 10 ), drain flies or fruit fries and several house flies in dining area & women rest room, 5 days to correct.
