Note: On-site inspection conducted in response to Food Borne Illness complaint, which stated that complainant bought "beef with vegetable stir fry and egg roll lunch special at approximately 3:30 pm from China Express on 7/11/2015." Complainant stated that the food was eaten approximately 4.5 hours later and appeared to be "slimy." Complainant felt ill the next day. Manager stated that the only complaint he has received recently from a customer was that a food item was not correct and the customer came in to demand a refund. He did not issue a refund because the food had all been consumed. Manager states that he offered to make the customer new food, but that offer was refused. Manager states that no employees have been ill in the last two weeks. All foods indicated in the Food Borne Illness Report were found at proper temperatures. Recommend complaint be closed. SS.

The following containers of food were found stored without a lid or covering: white rice on chest freezer, broccoli, sprouts, and lettuce on top of the prep cooler, various containers of food in the bottom of the prep cooler. Provide a lid or covering.
Food whether raw or prepared, if removed from the container or package in which it was obtained shall be stored in a clean, covered container except during necessary periods of preparation or service

1) Both hand sinks in customer restrooms are peeling away from the wall. Reseal hand sinks to wall.
Equipment shall be sealed to the adjoining equipment or adjacent walls or ceilings if exposed to seepage or splash.

- Pre-flushed, scraped, soaked
Multiple food containers in walk in cooler have current date mark labels, but also faded date mark stickers that were not properly scraped off of container during wash, rinse, sanitize process. Ensure all food containers are thoroughly washed of all old stickers and food debris before reusing.
Equipment and utensils shall be pre-flushed or pre-scraped and, when necessary, pre-soaked to remove gross food particles and soil.
- Non-food contact surfaces clean
1) Three box fans on top of walk in cooler are heavily soiled. 2) Exterior handles on prep coolers and Coca Cola cooler are soiled. Clean and sanitize all non-food contact surfaces as often as necessary to maintain clean.
- Single service articles not re-used
Containers intended for single-use are reused in the prep cooler, including a baking soda container that is reused to store bamboo shoots.
Re-use of single-service articles is prohibited.
- Floors properly constructed, clean, drained
Floors beneath equipment and behind coolers are soiled in kitchen and dry storage area. Clean floors routinely.

Exterior ventilation system on roof was found to be heavily soiled. Clean on a routine basis, maintain clean.
Ventilation systems shall be installed and operated according to law and, when vented to the outside, not create an unsightly, harmful or unlawful discharge.
