On-site inspection conducted in response to complaint regarding unapproved construction/remodeling of establishment during operation. Upon arrival for inspection, the following was noted: 1) No hot water is available in the establishment 2) No restroom facilities are available in the establishment 3) Three compartment sink is not properly plumbed and is draining into a bucket 4) Entire dry storage and prep area has been demolished and dust is being tracked throughout establishment 5) Plans for remodeling have been requested multiple times (through email, phone calls, and on inspection forms) and have not been received or approved License #15-1121 has been suspended for this establishment until approval is given to reopen by the Health Department. A Plan Review and Plan Review fee of $200 must be received as soon as possible by this Department to proceed with construction/remodeling. Facility is planing on doing a remodel of facility while remaining open. Spoke to Chris (district manager) about submitting floor plans and plan of action to assure no food will be contaminated during remodel process to PCCHD as soon as possible. Facility must have at least one certified food manager at this establishment. All food handlers must have an ANSI approved Food Handler Certificate.
