Given paperwork for variance request for outside food service on the premises.

Numerous prepackaged foods located in reach-in cooler are not properly labeled. Provide proper labeling for all prepackaged foods. Foods that are pre-packaged in advance of retail sale shall bear the following information in English on its label: the common and/or usual name of the product

Shelving unit in small rear storage room is not sealed. Properly seal shelving unit. Multi-use equipment and utensils shall be made and repaired with safe, non-toxic materials, including finishing materials that is corrosion resistant, non-absorbent, smooth, easily cleanable, and durable under conditions of normal use.

Sever condensation building in both storage walk-in freezers. Repair piping and leaks in walk-in freezer. Rooms shall have sufficient ventilation to maintain them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes.

HACCP: Owner inquired about possibly selling meat to customer. She was informed that all meat must come from an approved source and be shipped and stored in appropriate packaging. Also, the acqusition of meat will increase the risk category of the establishment, and such plans will be need to be submitted and approved by the Health Dept before operations can start.
All frozen foods are frozen. Chlorine sanitizer is utilized in the establishment.

Numerous boxes of food stored on floor or walk in freezer. Store boxes and containers of food a minimum of six inches above the ground or on easily moveable dollies, racks or pallets to protect the food from splash and other contamination, and to permit easy cleaning of the storage area

Ceiling tile removed from ceiling due to leak in roof. Repair and/or replace ceiling tile. Walls, including non-supporting partitions, wall coverings and ceilings of walk-in refrigerating units, food-preparation areas, equipment-washing and utensil-washing areas, toilet rooms and vestibules shall be light-colored, smooth, non-absorbent and easily cleanable. Note: Leak has been repaired by serviceman. A new ceiling tile has be purchased due to water and repair damaged.

Walk in cooler holding at 45 degrees. The walk in cooler must be able to hold foods at 41 degrees or less. Corrected. Cooler was holding at 37 degrees at end of inspection.

Working and accurate thermometers are needed in all cooling units.

Coolers and shelving must be cleaned and sanitized before food is placed in the units.

Leak from drain in two bay sink.

Gaps present in front door, rolling door, and in back double doors. Eliminate gaps. Back double doors not self-closing. Provide self closing doors to enterances to the outside.
