HACCP: Handwashing
Facility has not paid 2012 license fee. Facility is in danger of losing the food and drink license if fee is not paid, and currently is operating without a valid food and drink license. Without a valid food and drink license the liquor license is also invalid.
Please notify this department when and if a change of ownership occurs.
- In-use food dispensing utensils properly stored.
Ice scoop stored with handle in ice. Ice-dispensing utensils shall be stored on a clean surface or in the ice with the utensil~s handle extended out of the ice.
- Food contact surfaces of equipment and utensils clean
Soda guns and holsters soiled. Interior of beer cooler heavily soiled. Interior of microwave soiled. Clean and sanitize.

No paper towels at handsink. A supply of sanitary towels or a hand-drying device providing heated air shall be conveniently located near each lavatory.

HACCP: Gum chewing cl- used as sanitizer
- Food protection during storage, prepartion, display, service, transportation
Interior of beer cooler has approx. 1 inch of liquid at the bottom. Beer bottles have a layer of scum on them from sitting in this water. Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
- Wiping cloths clean, used properly, stored
Wet wiping cloths stored on bar countertop. Moist cloths or sponges used for wiping food spills on kitchenware and food-contact surfaces shall be stored in a sanitizing solution between uses.
- Plumbing installed and maintained
Three compartment sink drain leaking into basement. Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
