- Install a 3 compartment sink with hot and cold water to each compartment and drain boards on both sides.
- (A) Clean and sanitize all equipment to be used for food storage, prep, or service. (B) Observed KNIFE with dried food residue on it. (C) Food contact equipment must be in good repair; Observed a GLASS LID was being used as a cutting board. (D) Buy an NSF grade cutting board.
- Have a licensed plumber connect the establishment's waste water pipes to the public sanitary sewer system and install the 3 compartment sink. Have proper paper work to verify with health inspector and plumbing inspector.
- Evidence of roaches observed. Eliminate the presence of insects by using any approved means.
- Use bar hand sink for handwashing only.

- Observed open contaner of mixed fruit plate in reach in cooler; along with open packages and food containers in same cooler. All items had no labels on them. Recommend to label all open food packages and containers with: Food name, Date of prep/opening, and use by date.
- Observed heavy build up of dried urine and bodly waste in both reastrooms. Recommend to wash, rinse, and sanitize both reastroom doors, floors & walls. This is to be done daily. Recommend to recaulk around both hand sinks in both restrooms.
- Observed no paper towels in both restroom. Recommend to install paper towel holder with paper towels in side both restrooms.
- Observed crockpot and table warmer unit with old food in them, that was not discarded from last night's event. Recommend to wash, rinse, & sanitize all food service equipment before closing. This will prevent the attraction of rodents and insect to your facility.
- Reach in cooler without a inside thermometer. Recommend placing thermometers in all cooler that have food items in them.
- Observed crockpot and table warmer unit with old food in them, that was not discarded from last night's event. Recommend to wash, rinse, & sanitize all food service equipment before closing. This will prevent the attraction of rodents and insect to your facility.
