- 229.167 (e) (3) (A) no towels. One of the hand washing sinks did not have paper towels supplied to it. Ensure that all hand washing sinks are supplied with paper towels or an automatic hand dryer.
- 229.165 (k) (14) (C) quat solution. The quaternary ammonium compound solution used to sanitize dishes was too concentrated. Ensure that any quaternary ammonium compound solution used to sanitize food contact surfaces is at the manufacturer?s recommended concentration.

- Company's SOPs call for prepared pizza sauces to be held for only 5 days once prepared from concentrate; containers that sauces are being held in are not being marked with a use by or prepared on date. Clearly date label these containers and follow company SOPs.
- 229.166 (i) (1) (A)HWS not accessible. Two of the hand washing sinks had large containers in their basins. Handwashing facilities must be fully accessible at all times for employee use. There can be no barriers and these sinks can be used for handwashing only.
- 229.167 (p) (11) (C)insects/pests not minimized. Observed several gnats in food prep and storage areas. Eliminate the presence of insects by using any approved means.
- 229.165 (m) (1) (A)not clean. Several of the pizza pans had burnt on food stuck on them. Thoroughly clean or replace these food contact surfaces.
