  • Observed a few roaches on floor and live ones on sticky traps. Ensure that areas are cleaned and sanitized. Ensure all holes and gaps are sealed with approved materials. If using foam, foam must be cut to be even with surface and then sealed/painted. Clean and sanitize all areas where roach feces are found. Save pest control service receipts for verification.
  • Discontinue using household grade pest control chemicals in food establishment.
  • Ensure to clean holder for ice scoop. Ensure that inside reach in cooler is cleaned and all moisture and dead roaches are removed. Ensure that shelving where clean wares are stored is clean to sight and touch.
  • Ensure that most recent inspection report is posted in public view.
  • Prepared foods stored for more than 24 hours must be date marked. Date markings should state the expiration date of the food. The expiration date should be 7 days, with preparation day counting as day 1.
  • Do not store any items in hand sink for any length of time. Hand sink is for hand washing only.

  • 229.164 (f) (2) (A) (iv) not covered. Observed deli meats in container without a protective covering and refrigerator has a leak inside unit. Ensure food is stored with a protective covering such as foil, plastic wrap, or a lid.
  • 229.164 (o) (7) (A) consume by date (prepared). Observed chicken salad with a date lavle of the prep date and date was more than 24 hours prior. Ensure prepared food that is kept for more than 24 hours has a date label consisting of the USE BY date. The USE BY date shall not exceed 7 days from the prep day and the prep day will count as day 1.
  • Observed dead roaches throughout kitchen. Ensure dead roaches are eliminated and kitchen is monitored for activity. If activity is observed, use necessary and appropriate means to eliminate pests.
  • Observed gaskets for ice machine and refrigerator detached from door/lid. Ensure gaskets are repaired or replaced so they are full attached to door/lid.
  • 229.165 (m) (1) (B)grease and soil accumulation. Observed heavy grease accumulation at stove/fryer area between equipment, on floor, wall, and vent hood. Ensure grease is eliminated.

  • Establishment had been treated four days earlier, however roaches were observed in the establishment. The pest control company receipt does not include methods of treatment or locations treated and other information on the receipt is not legible.
  • Clean the inside of the ice machine.
  • Chest freezer door panel and outer gasket must be in good repair and be smooth, easily cleanable and non-absorbent.

  • Install an air gap of two times the diameter of the drain line between the ice machine drain and the sewer line.
  • Utensils must be in good repair
  • Clean the knife storage container.
  • Lids of the chest freezers must be smooth, easily cleanable and non-absorbent.
  • Clean the seals on the chest freezers.
  • Clean the ice machine lid.
