- Hand sink is for washing only. Ensure that hose is removed from hand sink in restroom.
- Observed several popsicles and ice creams without labels. Items that come from a larger package that states not for individual sale and also without out state requirements for labels may not be sold individually.
- Ice bags need to have state requirements for labels.
- Observed several packets of Pioneer biscuit mix that was old and packaging was severely faded with water damage. One packet had tape on bottom to keep it together. Food must be honestly presented, safe and unadulterated.
- Ensure to remove mold from inside ice machine. Ice machine insulation on door needs to be replaced. It is cracked and separating from lid.
- Provide soap and paper towels at all hand sinks.

- Eggs were stored above ready to eat food and beverage items. Eggs should be stored below ready to eat foods/beverages to prevent possible cross contamination.
- 229.167 (e) (3) (A) no towels. Hand sinks did not have paper towels. All hand sinks should have paper towels at all times.
- Thermometer in walk in cooler and cold hold unit by front counter were not reading the internal cooler temperature accurately. Advised establishment Representative to obtain thermometers that will accurately read the temperatures.
- Several dead large roaches were seen through the establishment: In back storage room and on the side of the ice machine cooler. Clean throughly to remove roaches.
- Observed chemical pesticide intended for home use. Remove home pesticide. Commerical pesticide intended for a food establishment should be used only.
- Internal surface of ice machine had a black mold/mildew build up on the surface. Clean ice machine to remove build up and prevent conatamination of ice.

Also all food items sold in establishment must have a proper manufacturer label on them. Package children drinks being sold in establishment do not have proper labels on them. Because, they were not designed to be sold seperately, but as a whole, in original package box.
