  • Observed bottle water on shelf over dishwares and personal food items on prep table. Observed personal plant in walk in cooler on same shelf as food items. Ensure that all personal food items, such as beverages and personal foods, are stored in an area away from food and are stored in a cup with a lid and a straw to prevent cross contamination. (Bottled and open canned beverages should not be used).
  • Provide paper towels at all hand sinks.
  • Install an air gap of two times the diameter of the drain line between the ice machine and the sewer line.
  • Provide for adequate backflow prevention device on any threaded hose bib for mop sink.
  • Replace broken lids to bulk food items.
  • Ensure that outside of bulk food containers and knife holder is clean to sight and touch.
  • Remove mold inside ice machine.

  • Observed cold hold foods items on front service line at 46 to 50 degrees F. Foods needing to be under cold hold temperatures should be maintained at 41 degrees F and below. Ensure that ice is touching bottom of metal containers to help maintain temperatures of 41 degrees F or below.
  • Discontinue using broken, chipped or melted dishwares. This includes lids to bulk food items.
  • Ensure that all food contact surfaces are clean and in good condition such as mold in ice machine, cleaning around soda spouts, lids to bulk food items, etc.

  • Observed lids to spices/rice/beans/etc broken and missing pieces. Bean lid is cardboard wrapped in plastic wrap. Coffee maker has broken brew basket is also broken. Discontinue using broken/chipped/melted dishwares. Ensure that food grade and easy cleanable lids are used.
  • Ensure that all food contact surfaces are clean and in good condition such as area around soda spouts and ice shoot, above ice machine opening, etc.

  • Ensure that all food contact surfaces are kept clean and in good condition such as microwave, shelving holding clean wares, ice scoop holder, ect.
  • Repair the leak at ice machinen that drains directly onto the floor.

  • Food contact equipment must be in good repair- cutting boards are deeply scarred- replace.

  • Observed chunks of beef fajita in hand sink located by the drive thru area. Ensure the hand sink is used for hand washing only and not for other activities, such as disposing of food.
  • The mop sink has two faucets that are connected with a hose. The left faucet does not have a back flow preventer. Ensure the mop sink faucet is equipped with a back flow preventer.
  • Observed drain pipe from the hand sink by the drive thru area without an air gap to the floor drain. Ensure there is an air gap that is 2x the diameter of the pipe between end of the pipe and top of the drain. Corrected on site.
  • Observed cold hold unit across from grills without a thermometer. Ensure cold hold unit is equipped with a thermometer.
  • Observed lid of the rice bin cracked. Ensure lid is in good repair, smooth, and easily cleanable.

  • Observed inside of microwave by drive thru window soiled with food debris. Ensure microwave is clean to sight and touch.

  • 229.165 (n) (1) (C)at least every 4 hrs. Observed utensils at grill station and near cold hold unit that were soiled with food debris and had been there since the morning. Ensure utensils that are used to cut/dispense/serve potentially hazardous food are properly washed/rinsed/sanitized every 4 hours.
  • Observed container being used to store wares at grill station soiled with food debris. Ensure container is clean to sight and touch.
