- Please ensure potentially hazardous food items and cold hold units are maintaining a temperature of 41 degrees F or below. During inspection reach-in cooler was at 56 degrees F. All PHFs were removed from cooler and relocated to WIC.
- Please ensure wastewater plumbing system is installed according to code. An air gap between the ice machine drain pipe and the flood level rim of the floor drain shall be at least twice the diameter of the drain pipe and may not be less than 1 inch. Found that air-gap was not present at ice machine drain point.

- 229.163 (n) (1) eat.drink.smoke. Open employee drink found stored on clean wares rack near dishwasher. Employees must drink from clean closed beverage containers with cleans hands and away from clean utensils and food prep areas.
- 229.164 (o) (7) (A) consume by date (prepared). Refrigerated, ready-to-eat food not properly marked with a use by date. Refrigerated, ready-to-eat, PHF foods held for more then 24 hrs. shall have a consume-by-date. The day of preparation shall be counted as day 1. If held at 41F or less food can be kept for a max of 7 days at 45F a max of 4 days.
- 229.166 (k) (1) (B) approved sewage disposal. Drains in kitchen are slow and when dishwasher drains, wastewater backs up into kitchen. Drain not working properly in area with ice machine, handsink drains slowly in same area. Establishment has 24 hours to repair plumbing so that drains from ice machine and kitchen do not back up. If repairs to plumbing cannot be made so that wastewater does not pool into kitchen, establishment's food license will be suspended and establishment must close until repairs are made.
- 229.165 (m) (1) (A)not clean. Drawers where utensils and food preparation items are held were found unclean with food debris, grease and other build-up. All equipment and food contact surfaces must be kept clean.
- 229.164 (o) (9) (A) (iii) unmarked food>4 hrs. Time only held potentially hazardous food not marked or identified to indicate time food was removed from the temperature control. PHF held out of temperature control must be discarded within 4hrs.
