- Observed pizza boxes on floor in dry storage closet. Please ensure all food contact surfaces are 6 inches off the floor.
- Observed popcorn seasoning stored on cardboard container with paper towels placed on bottom of cardboard. Please store food items on surfaces that are easily cleanable, smooth and non-absorbent.
- Observed missing metal door below deep frying unit where food debris has built up on internal parts.
- Found wiping clothes stored on counter. Please store in sanitizer pail between uses.

- The cabinets and counter tops in the kitchen are worn. The cabinet doors have become sticky. The cabinets by the three compartment sink on the right side have become water logged at the base.
- Please clean the ice shield in the ice machine. Black mildew has built up.
- Non food contact surface cleaning: handles to cabinets in kitchen have build up on them, dust fans in kitchen, clean char build up off the range top, defrost the pizza freezer by the bar area, dust fan unit to walk in cooler, clean condensate off the floor of the beer cooler on south side of bar, clean both pizza oven catch trays. Clean as needed.
- Found single service forks with food contact surface presented. Make sure handles are all going the same direction.
- The floor in the walk in cooler has become worn and no longer easily cleanable.
- Please clean the dust off the fan unit and the ceiling in the walk in cooler. Dust ceiling by the vents in the kitchen.
- The wall by the mop sink has become worn and no longer easily cleanable.
- Fix burnt out lights in kitchen and replace shield.
- Found cooled down soups in freezer. If facility wants to cool down a change in category and HACCP plan must be complete.
