- The reach in freezer under the sandwich topping area has rusty racks. There is paint coming off one of the racks in the storage area for dishes.
- Wash, rinse, and sanitize the following: the shelves in the dry storage area, seals (to the salad cooler, plate cooler, reach in freezer), the front of the Flav R Savors (that hold biscuits and the one under pancake batter), the microwave handle over the sandwich toppings area, under the grill line, and in front of the grill line around the dials, under the ice storage bin in the wait staff area. There was also a hair by the tomatoes in the sandwich topping area.
- There is a drip above the soup prep area.
- There is buildup of debris on floors in the back area by dry storage, the grill line, and in the dish machine area.
- There is buildup on walls in the potato/dough area and in the dish machine area.

- Observed multiple employees improperly washing their hands. They all had touched the handles of the faucet, instead of using a paper towel. Both employees on the grill line had the rubber bracelets on their wrists, one of which had a couple. One employee pulled up his pants, touched a handle, and used a rag to clean then going to food prep. Also in the dish machine room the employee went from handling dirty dishes to putting away clean dishes without any hand washing. It seems that employees need more training on hygiene and hand washing. I want you to conduct a training going over proper hand washing, when to wash hands, and proper hygiene. I need documentation of training and verification all employees know how to properly wash their hands by 8-7-15.
- The lid storage rack is rusty and there are racks that are missing paint in the container storage area.
- There are several pieces of equipment throughout the kitchen that need cleaned, especially the microwaves, dry storage containers, and shelving. In the container storage area there were several containers that still had date labels on them. You need to make sure they are removed prior to storage. There are also other surfaces that have black buildup due to moisture accumulation. The facility needs to implement a cleaning schedule. If you already have one, you need to make sure that it is actually being done.
- There were multiple containers in storage that were stacked wet.
- Under the 3-compartment there was a steady drip of water coming from pipes. The water was accumulating on the floor due to not having a drain underneath.
- Floors throughout the facility are dirty.
- The walls throughout the facility are dirty. In the vents in the ceiling there is moisture accumulation which is causing a black buildup.

- The lids for the ice under the soda fountains are left open. These must be closed when not in use to help prevent the ice from being contaminated due to it being a food.
- The reach in freezer under the sandwich toppings is missing a piece of plastic. There is paint coming off the blue drying racks. The racks in the dish room are rusted.
- Prep surfaces, shelving, and insides of reach in coolers and freezer had a build up of debris.
- The Flav R Saver and equipment on grill line had an accumulation of grease and food buildup.
- The handles and seals to coolers and freezers had an accumulation of food buildup.
- There are missing floor tiles outside the walk in freezer and around the mop sink.
- There is coving missing by where employees' lockers are.
- There is buildup on floors, especially in food prep areas.
- The facility only has 1 person with their food manager's license. Other people have their Serv Safe license, but not IL food manager's license. Discussed with manager that there needs to be someone present at all times with their IL food manager's license. The employees who have their Serv Safe license will need to contact IDPH to get their IL license. Once they have submitted the paperwork for this contact me.

- The metal dish racks in the dish room are getting rusty. Paint or replace them so they are easily cleanable.
- Kitchen counters and insides of coolers and freezer had a build up of debris. Clean and sanitize them.
- The insides of the Blodgett ovens, grills had a build up of food debris. Clean and sanitize them.
- The outsides of the ovens and cooler handles and cooler door seals had a build up of debris. Clean and and sanitize them.
- Found build-up of debris on floors. Clean the floors.
